You might give hypnosis a try - as a biologist I was always VERY skeptical about hypnosis having any value but a nasty bout with depression made me take another look at it as a remedy. It really worked for me, and also appealed to my generally lazy nature in that all I had to do was listen to a cd once a day.
It probably won't make you quit cold turkey, but it should help you learn deep relaxation techniques - which should help you keep from climbing the walls when you get cravings and make it easy to hang on to your willpower.
Maybe get out and just do some easy snorkeling - you might get lucky and spot a huge fish which would be a hell of an incentive to get healthy so you can go after them!!!
