Freedive Medical Form

Thread Status: Hello , There was no answer in this thread for more than 60 days.
It can take a long time to get an up-to-date response or contact with relevant users.

Does anyone have a good "fit to freedive" medical form I can take to my doc?

I have my annual commercial dive medical tomorrow and want to get signed off for next year's comps while I am there.

I have the form used for Hawaii, Cyprus and my own comp but most doctors think this is pretty useless. I also have the RSTC Medical Statement but this does not specifically mention freediving
