Dear Diver,
I am writing to invite your participation in a research survey. This survey looks at scuba diving practices and any injuries that resulted from diving. I am an Emergency Medicine resident and an avid dive enthusiast, and am asking scuba divers from the United States to respond. This survey will take about 10 minutes of your time.
Through this research, I hope to identify common dive injuries, identify the type of divers that become injured, and their knowledge of diver safety practices and preferences. I hope to provide all divers, as well as EM physicians, with this information and better prepare them to treat dive-related injury and illnesses at the dive site and when you return home.
This project was approved by the Institutional Review Board, the committee that oversees research at this institution, at the Resurrection Medical Center in Chicago, Illinois. Participation is voluntary; refusal to participate will involve no penalty or loss of benefits to which you are otherwise entitled. Returning the completed questionnaire implies full consent to participate.
If you are willing to help, please complete the questionnaire and return it as directed. One option at the end (unlinked to responses) provides space for an e-mail address entry for notification of results if you would like a copy.
Your assistance with this project is greatly appreciated. If you have any further questions or need more information, please feel free to contact me. Thank you so much for your time and help.

Took your survey, a problem that I see is that you limit the responses and it will skew your results, I found that about 30% of my responses needed additional information that it would not let me give and about 10% forced me to give an answer that was close but not totally correct.
